REQUESTING COPIES OF A  VOLUNTARY DECLARATION OF PARENTAGE

If you are looking to obtain certified copies, you will need to:

1. Submit a request form
   • DCSS 0918 Request for a Certified Copy of a Filed Voluntary Declaration of Parentage (English)
   • DCSS 0918 Request for a Certified Copy of a Filed Voluntary Declaration of Parentage (Spanish)
2. Email your request and attach a legible photo of your valid government issued ID to askpop@dcss.ca.gov.

Upon receipt, the request will be processed within 2-5 business days and mailed via USPS to the address provided on request.
