REQUESTING COPIES OF A  VOLUNTARY DECLARATION OF PARENTAGE

If you are looking to obtain certified copies, you will need to submit a request form (DCSS 0918).

Please email your request and a legible photo of your valid government issued ID to askpop@dcss.ca.gov.

Upon receipt, the request will be processed within 2-5 business days and mailed via USPS to the address provided on request.
